Family Medicine Physician Job at Premise Health in Greeley, CO

Premise Health is seeking a Part-Time Nurse Practitioner to provide patient-centered care at our employer-based Health Center in Greeley, CO. Join a mission-driven organization focused on helping people get, stay, and be well—while maintaining excellent work-life balance.

Schedule

• 8 hours/week

• Fridays | 7:00am–4:00pm

Compensation

• $58.50 – $65.00 per hour (based on experience and qualifications)

Position Highlights

• Deliver primary and preventive care to employee populations and their dependents

• Perform exams, procedures, and basic acute care (injections, suturing, wound care, infection management)

• Develop and manage patient care plans

• Order and interpret diagnostic testing

• Collaborate with physicians and multidisciplinary clinical teams

• Educate patients on treatment plans and health maintenance

• Maintain accurate, timely medical documentation

Required Qualifications

• Active CO RN and Nurse Practitioner license

• National NP certification (ANCC or AANP)

• DEA license required

• Minimum 1 year NP experience

• Master’s degree in Nursing

• BLS certification (ACLS may be required based on contract)
