We are seeking a patient focused Nurse Practitioner or Physician Assistant to join our dedicated team in Palliative Care. The ideal candidate will have excellent communication, critical thinking skills, and be a proactive team-player. Our team focuses oncare that meetspatients and family where they are in their journey.Wemeet daily to debrief about patients and seek guidance from each other as needed.

Position Details:

• Full-time, Day Shift
• 8:00am-4:30pm (flexible as needed with on-call rotation)
• Rotating monthly after-hours call
• Practice with 2 Physicians
• 100% inpatient

Key Responsibilities:

• Direct patient assessments including: H&Ps, discharge summaries, per visit documentation, medication/symptom management, interdisciplinary team participation.
• Lead goals of care conversations with patients and families
• No procedural requirements

Qualifications:

• Master's Degree required
• ACNP, FNP, PA, PMHNP,or WHNP certification required
• Ideal candidate will have: Palliative/Hospice, ICU, Oncology, or Critical Care experience as an RN or APP
