Team Lead- Nurse Practitioner or Physician Assistant- Virtual Care- Hospital at Home & On-Demand Urgent Care

Job Description

Indiana University Healthis seeking an APP Team Leadto join our innovative virtual healthcare team. The selected candidate will flex between our On-Demand Urgent Care and Hospital at Home teams.

Position Details:

  • Full-Time (1.0 FTE)
    • Rotating Days/Weekends
  • 50% Outpatient & 50% Inpatient
  • Leveraging virtual platforms to care for patients requiring acute hospital level care and low acuity urgent care from a virtual provider setting
  • Collaborate with the IU Health Emergency Medicine department and/or inpatient disciplines to identify appropriate patients for care and facilitate their transition to home
  • Perform virtual admission visits with an in-person nurse using remote physical exam technology and create a customized care plan with individualized goals to determine future readiness for discharge

Practice Location:

  • Hybrid schedule - Remote work from home, with training and occasional time spent at our virtual care office in downtown Indianapolis. All care is provided virtually.

Team Lead Responsibilities:

  • Assist with planning and directing the operational activities as outlined by the organization
  • Oversee adherence to organizational policies and proper communication to and involvement of subordinate personnel
  • Support the Advanced Practice Provider in collaboration with their practice administrator and physician leadership

Qualifications:

  • ACNP or FNP certification required
  • BE/BC
  • At least 1 year of APP experience required
  • Urgent Care, Family Practice, Internal or hospital medicine experience is preferred
