Indiana University Healthis seeking an APP Team Leadto join our innovative virtual healthcare team. The selected candidate will flex between our On-Demand Urgent Care and Hospital at Home teams.
Position Details:
- Full-Time (1.0 FTE)
- 50% Outpatient & 50% Inpatient
- Leveraging virtual platforms to care for patients requiring acute hospital level care and low acuity urgent care from a virtual provider setting
- Collaborate with the IU Health Emergency Medicine department and/or inpatient disciplines to identify appropriate patients for care and facilitate their transition to home
- Perform virtual admission visits with an in-person nurse using remote physical exam technology and create a customized care plan with individualized goals to determine future readiness for discharge
Practice Location:
- Hybrid schedule - Remote work from home, with training and occasional time spent at our virtual care office in downtown Indianapolis. All care is provided virtually.
Team Lead Responsibilities:
- Assist with planning and directing the operational activities as outlined by the organization
- Oversee adherence to organizational policies and proper communication to and involvement of subordinate personnel
- Support the Advanced Practice Provider in collaboration with their practice administrator and physician leadership
Qualifications:
- ACNP or FNP certification required
- BE/BC
- At least 1 year of APP experience required
- Urgent Care, Family Practice, Internal or hospital medicine experience is preferred
