Geriatric Medicine Job at First Physicians Group in Sarasota, FL

Sarasota Memorial First Physicians Group is seeking a Certified Physician Assistant or Advanced Practice Registered Nurse to join our Post-Acute Team.

The primary function of the PA |NP role is to provide evidence-based, compassionate, and high-quality direct patient care with a focus on post-acute care. The PA | NP should be comfortable working both independently in the field and in collaboration with other members of a multidisciplinary team in nursing home and assisted living facility settings.

Opportunity Highlights:

• Monday – Friday office hours are 8:00am-4:30pm
• Provide post-acute services in skilled nursing facilities for both sub-acute and long-term care patients.
• Visits include new patient admissions, follow-ups, acute visits, discharges, pre-and post-operative, as well as long-term regulatory visits.
• Provide patient/family education as needed.
• Order laboratory and diagnostic testing and interpret results.
• Order, manage and review medications and coordinate with facility staff.
• Coordinate treatment and makes appropriate referrals to other health providers as necessary.
• Manage tasks, patient call backs and communicating results.
• Equitable call for nights and weekends.

Requirements: 

• Graduation from an accredited master’s degree program
• Current board certification
• Active or eligibility for licensure by the State of Florida
• BLS
• DEA Certificate
• Ability to work independently.
• Post-acute or RN Critical Care experience preferred
