About the Role
As a Careway Physician, you will play a critical role in our care model by conducting virtual intake visits, laying the clinical foundation for our members' ongoing health journeys. The intake visit includes establishing care, clarifying diagnoses, uncovering barriers, and partnering closely with our Health Advocates to convert individualized goals into concrete, coordinated next steps. As this is a unique new model of care, we will also value your input in the designing and refining of the model to ensure Careway can serve a vulnerable population with empathy, clarity, and results.
How You Will Make an Impact:
● Conduct comprehensive telemedicine-based evaluation and management (E&M) visits to establish care, evaluate medical conditions, identify barriers, and initiate care plans.
● Partner closely with Careway’s Health Advocates, providing the clinical context and guidance they need to support whole-person care that integrates medical, emotional, and social needs.
● Review and oversee individualized care plans developed by Health Advocates, ensuring clinical alignment tailored to each patient’s unique needs.
● Document assessments and clinical findings clearly and efficiently in our EHR in accordance with Medicare billing standards and telemedicine best practices.
● Mentor and guide Health Advocates as they coordinate appointments, secure records, and coach patients toward greater health literacy and self-management.
● Shape our care model by sharing frontline insights with product, data, and operations teams so we can iterate quickly and serve patients even better.
Note: This role is strictly non-prescribing. You will not adjust medications, order labs, or provide ongoing disease management. Your focus is on assessment, coordination, and advocacy collaboration.
What You Will Bring:
● Currently located in California.
● Active MD or DO license in California, in good standing.
● Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred; other relevant board certifications with significant experience caring for older adults will be considered (e.g., Emergency Medicine)
● Experience delivering patient-centered care to Medicare beneficiaries and/or older adults.
● Expertise in understanding complex, multifactorial health issues in geriatric and adult populations, including their social determinants.
● Tech-savvy and comfortable adopting new EHRs and remote-collaboration tools.
● A collaborative spirit and passion for delivering whole-person care in a virtual, interdisciplinary team environment.
● Active enrollment as a Medicare provider (preferred).
Nice-to-Haves:
● Multi-state or IMLC compact licensure.
● Background in value-based care, health coaching, or community health work.
● Spanish or other language fluency.
● Interest in clinical informatics, product design, or medical education.

● Status: 1099 Independent Contractor.
● Schedule: Fully remote; minimum commitment of 10 hours/week.
