Job Title: Integrated Behavioral Health Counselor  

Reports to: Director, Value Based Programs

Status: Full-time, Exempt

We’re seeking a compassionate, driven Integrated Behavioral Health Counselor (IBHC) to join our growing interdisciplinary team and help bring behavioral health support directly into the primary care setting.

In this role, you’ll play a key part in our SBIRT (Screening, Brief Intervention, and Referral to Treatment) program—screening for mental health and substance use concerns, providing brief interventions, and connecting patients to continued care when needed. You’ll collaborate closely with primary care providers to ensure patients receive timely, compassionate, and coordinated care.

Job Responsibilities:

As an IBHC within Duke City Primary Care, day-to-day duties may include, but are not limited to, the various tasks below:

  1. As part of an integrated care team, the IBHC provides brief screening of substance abuse and mental health issues for patients presenting in clinic for primary care services including:
    1. Healthy Life Questionnaire (HLQ)
    2. Drug Abuse Screening Test (DAST – 10)
    3. Patient Health Questionnaire (PHQ-9)
    4. Posttraumatic Stress Disorder Checklist (PCL-5)
    5. Generalized Anxiety Disorder (GAD-7)
  2. Performs brief interventions and brief treatments for patients qualifying for services as part of SBIRT including:

  1. Motivational Interviewing
  2. Harm Reduction
  3. Seeking Safety
  4. Suicide Prevention
  • Completes clinical care documentation to include but not limited to: results of initial screeners, progress notes, and referrals as needed per time limited requirements.
    1. Provides warm hand-offs to Behavioral Health clinic line for any patients requiring more intensive services.
    2. Timely enters and completes appropriate clinical documentation per industry and agency standards and holds oneself accountable for his/her actions with regard to proper coding.
    3. Appropriately charging for all services rendered including accuracy, appropriateness, and providing care to clients founded on evidence-based practices.

    1. Works closely and communicates well with the documentation specialists who will assist in charting tasks and other closely related duties to better maintain efficiency.
    2. Works collaboratively with the PCP and other interdisciplinary team members to ensure patients screened as positive are engaged in SBIRT services within the required timelines.
    3. Sees patients in a timely and efficient manner as scheduled and responds to patients in between scheduled appointments through appropriate and timely methods.
    4. Other duties may be assigned based on clinical/programmatic need.

      Education, Experience and Licensing Requirements:

    1. Possess a Master’s or Doctoral degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE) OR
    2. Possess a Master’s or Doctoral degree in marriage and family therapy from a school fully accredited by the Commission on Accreditation for Marriage and Family Therapy Education (COAMFTE) or a regionally accredited institution OR
    3. Possess a Master’s or Doctoral degree in counseling from a school fully accredited by the Council on Accreditation of Counseling and Related Education Programs (CACREP)
    4. 3 years directly related experience preferred.
    5. Competent in all Microsoft Office applications (up to intermediate skill level as a minimum
    6. Possess a full and unrestricted license or certification by the state of New Mexico to independently practice social work at the Master’s degree level OR
    7. NM State licensure as an independent Professional Counselor OR
    8. NM State licensure as an independent Marriage and Family Therapist
    9. Possess CPR Certification for Healthcare/BLS Providers or Professional Rescuer for employees working on site.

      Physical Demands Requirements:

    1. Sedentary Work: Able to sit for up to 6 hours per day; lift up to 10 lbs. occasionally during a day.

    Behavioral Demands/ Requirements: 

    • Skilled in behavioral health assessment, diagnosis, counseling, and referrals.
    • Proficient in crisis assessment and transfer to higher levels of care when needed.
    • Knowledgeable in evidence-based practices including Motivational Interviewing, Harm Reduction, and ICD-10 diagnoses.
    • Demonstrates sound clinical judgment, ethical practice, and adherence to HIPAA and regulatory standards.
    • Maintains timely documentation and meets all clinical and operational requirements.
    • Works independently and collaboratively; adapts to change and proposes solutions.
    • Builds strong relationships with clients, families, and staff; promotes a positive work environment.
    • Provides feedback to leadership on clinical and operational improvements.
    • Models professionalism, accountability, compassion, and quality care.
    • Supports and helps train staff; committed to learning and implementing new technologies.
