Advanced Practice Provider, Palliative Care

Job Description


Overview

The Palliative Care Advanced Practice Provider (APP), including Advanced Practice Registered Nurses (APRNs) and Physician Assistants (PAs), is an integral member of the interdisciplinary palliative care team. The APP is responsible for providing comprehensive, compassionate care to patients with serious, life-limiting illnesses in inpatient settings. Responsibilities include assessment, diagnosis, symptom management, and development of care plans within the APP’s scope of practice.

The APP collaborates closely with physicians, residents, nurses, and other healthcare professionals to ensure high-quality, patient-centered care that emphasizes symptom relief and alignment of care with patient and family goals. The provider demonstrates service excellence, professionalism, and accountability, including awareness of the relationship between resource utilization, access to care, and overall quality outcomes.

  • Provide specialized palliative care services, including symptom management, and advance care planning.
  • Perform assessments, formulate diagnoses, and implement treatment plans for patients with complex medical conditions.
  • Collaborate with the interdisciplinary palliative care team to support holistic care addressing physical, emotional, social, and spiritual needs.
  • Maintain accurate, complete, and timely medical documentation for all patients.
  • Assist in coordination of care across settings including inpatient units, outpatient clinics, home-based programs, and long-term care facilities (as applicable).
  • Communicate effectively with patients, families, and healthcare team members to promote shared decision-making.
  • Provide education and support to patients and families regarding disease progression, treatment options, and palliative care principles.
  • Participate in quality improvement initiatives that advance palliative care practice and service efficiency.

Qualifications

Qualifications

  • Education: Master of Science in Nursing (MSN) or a Masters of Physician Assistant Studies (MPAS)
  • Current and valid Physician Assistant License or Advanced Practice Nurse License in the State of Illinois or eligibility for licensing
  • Basic Life Support (BLS) certification required.
  • Federal DEA registration and State of Illinois Controlled Substance License required.
  • Strong critical-thinking and problem-solving skills.
  • Ability to deliver compassionate, patient-centered care to individuals with serious illnesses.
  • Experience in palliative care, hospice, oncology, gerontology, or related specialty is preferred.

  • Clinical base hourly pay range for this position: $55.20 – $63.65
    • Range shown should be used as an estimate and can be impacted by many factors including but not limited to the critical need of the position, overall experience and qualifications, community need, and other considerations. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.
