Role Overview

• Schedule: Monday to Friday
• Provide patient care in a skilled nursing facility by conducting rounds in their short-term units
• Conduct documentation at home with provided iPad laptop within 48 hours of seeing patients

Key Responsibilities

• Perform assessments and therapy sessions
• Collaborate with facility staff to enhance patient care
• Ensure timely and accurate documentation

Requirements

• Hold a valid full license as either a licensed professional counselor (LPC) or a licensed mental health counselor (LMHC)
• Experience in healthcare or long-term care is preferred but not required

Compensation
Expected compensation is estimated salary range of $81,000 - $96,000 with no cap on productivity income potential.
