POSITION SUMMARY
The Clinical Pharmacist is responsible for assessing patients, initiate and adjust medications, and monitor and treat disease states across multiple disciplinary fields.

ESSENTIAL SKILLS AND EXPERIENCE:
- Graduate of an ACPE – accredited School of Pharmacy with a PharmD degree
- Current Arizona State Board of Pharmacy License in good standing
- Certification to perform Cardiopulmonary Resuscitation for Health Care Professionals (CPR) and AED
- Valid Arizona Driver’s
- Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations
POSITION REPONSIBILITIES
- Advise on medication therapies for patients
- Ensure therapies are safe, appropriate, and cost-effective
- Evaluate and order pertinent laboratory work
- Refill, monitor, and adjust medications as needed
- Provide education to patients
- Discuss adverse effects, disease progression, and potential lifestyle changes during visit
- Ensure that patient understands how to appropriately take/use medications
- Counsel patients on medications and adverse effects
- Clinical consultations with practitioners, recommending therapies and monitoring plans as needed
- Address patient concerns
- Identify any medical conditions not addressed by primary care team
- Refer patient to appropriate available programs
- Consult with providers and nursing staff
- Medication reconciliation
- Medication therapy management
- Ensure cost-effective medication is available and in use
- Identify and provide resolution for instances where cost-effective medication is available to patients
- Education and communication with staff and providers on cost-effective medication and medical therapies when available
- Contribute to effective department operations
- Document financial impact of clinical activities
- Monitor performance
- Assist in writing and implementing policies and procedures
- Prepare reports as requested
- Travel to multiple sites to assist across all Adelante pharmacies
- Other duties as requested or assigned

- Bilingual (English/Spanish), preferred
