We’re currently seeking a LCSW to join our amazing team.

Details of the Pathologist opening:

• Anticipated start date: ASAP
• Anticipated schedule: Day 5x8-Hour (09:00 - 17:30)
  
• Length of Assignment: 18 weeks
  
• Are weekends required: Yes, 2 weekend shifts every pay period
• Expected Ratios: Varies
• Special requirements: Seeking Care Manager (Social Worker or RN) experienced in Hospital Care Management and discharge planning
• Type of Facility: Short Term Acute Care / Level IV Trauma Center
• Patient Age Group: Adults
• Total Staff Beds: 60
• Charting: Epic

Responsibilities:

• Conduct psychosocial assessments and develop care plans
• Support patients and familites through illness, trauma, and discharge
• Collaborate with interdisciplinary teams to ensure continuity of care
• Connect patients to community and post-acute resources 

What You’ll Need to Qualify for this Role:

• Current LCSW License 
• Two years experience in hospital or acute care setting
