Multifaceted APP Position with Hospitalist, In-Patient Rehab, and Hospice Responsibilites

Job Description

We are seeking a highly motivated and compassionate full-time Hospitalist Advanced Practice Provider (APP) to join our multidisciplinary team. The APP will provide comprehensive inpatient care across acute hospital services and inpatient rehabilitation, while also supporting patients and families through hospice and palliative care services. The ideal candidate is a strongcommunicator, team-oriented, and comfortable managing complex medical conditions across the continuum of care. Responsibilities

Acute Care (Hospital Medicine):

  • Perform initial history, physical examination, assessment, and daily management of hospitalized patients
  • Collaborate with physicians, nursing, and ancillary staff to develop and implement evidence-based treatment plans
  • Manage acute medical conditions, order and interpret diagnostic tests, and adjust care plans accordingly
  • Participate in rapid response, code situations, and transitions of care

Rehabilitation Medicine:

  • Provide ongoing medical oversight for patients admitted to inpatient rehab units
  • Monitor progress, address medical complications, and optimize chronic condition management during rehabilitation
  • Collaborate closely with physical, occupational, and speech therapists to support functional recovery goals

Hospice & Palliative Care:

  • Deliver symptom management and supportive care for patients with serious or life-limiting illnesses
  • Conduct goals-of-care conversations with patients and families
  • Partner with palliative and hospice teams to coordinate smooth transitions and holistic patient support

Schedule

  • Full-time position with a combination of daytime inpatient hospitalist and rehab service coverage.
  • Rotational weekend /holiday coverage as assigned

Qualifications and Licensing Requirements

  • Master's Degree required
  • Graduate of accredited APRN or PA program
  • Minimum of 1-2 years inpatient or hospital medicine experience preferred
  • Rehabilitation, hospice, or palliative care experience strongly preferred
  • Current Kansas license
