  • Schedule: M-TH 7am to 5:30 pm
  • Practice Overview: Providing face to face comprehensive medication management; Offering virtual CMM services across our Middle TN region; Building on our existing hospital-based Pharmacotherapy Clinic inspired by a 2023 ASHP Best Practice Award winner.
  • Facility: Ascension Saint Thomas River Park Hospital will be the base location
  • Location: Nashville, TN

Responsibilities

Assure the appropriate selection, dosing, administering, and dispensing of medication orders. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.

Job Duties:

  • Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
  • Recommend formulary alternatives when appropriate.
  • Compound and dispense medications for accurate delivery to patients.
  • Provide drug information and education to medical staff and patients.
  • Communicate with physicians and other clinicians as necessary to facilitate care.
  • Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
  • Assist with academic functions such as serving as a preceptor for pharmacy students.
  • May float to provide coverage of pharmacy needs.
  • Provide patient counseling when required.
What We Are Looking For
  • A passion for serving rural and underserved patient populations and a deep commitment to improving health outcomes in these communities.

  • PGY-2 Ambulatory Care residency training or a minimum of three years of equivalent, full-time ambulatory care pharmacy experience.

  • Proven experience practicing under a collaborative practice agreement, including prescribing or modifying medications autonomously within a defined scope.

  • Excellent communication and interpersonal skills, with the ability to build rapport with both patients and a multidisciplinary healthcare team.

  • A self-starter with a proactive and innovative mindset, capable of working independently to manage a patient caseload and identify opportunities for program improvement.

  • Strong clinical knowledge and critical thinking skills to provide comprehensive medication management and solve complex pharmacotherapy issues.

  • Experience with electronic health records (EHRs) and telemedicine platforms.

  • Board certification (e.g., BCACP) is highly preferred.


Requirements

Licensure / Certification / Registration:

  • Pharmacist credentialed from the Tennessee Board of Pharmacy obtained prior to hire date or job transfer date required.

Education:

  • Bachelor's or advanced degree in Pharmacy required.
