About the Role:

We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our care team in Greensboro, NC. As a Pod Provider, you will deliver high-quality, person-centered care with a strong focus on chronic disease management, behavioral health, acute care, and preventative services. You will work collaboratively within a multidisciplinary team to create and implement care plans that meet the individual needs of our members.

Key Responsibilities:

• Provide direct care for patients with chronic conditions such as hypertension, diabetes, and kidney disease

• Evaluate and manage acute concerns across all age groups

• Support behavioral health care, including depression, anxiety, and substance use disorders

• Deliver preventative care, including sexual and reproductive health services

• Conduct initial clinical intake visits and develop ongoing care plans

• Perform timely follow-up after hospital discharges, ED visits, or other transitions of care

• Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care

• Contribute to the development of clinical programs and continuous quality improvement initiatives

Qualifications:

• Active and unrestricted NP or PA license in the state of North Carolina (required)

• Minimum of 3 years clinical experience, preferably in primary care or chronic disease management

• Strong clinical judgment and communication skills

• Commitment to delivering equitable, team-based care
