Job Description

MAJOR DUTIES - Applys biopsychosocial evaluations to propose behavioral health interventions. - Provides evidence-based treatment, including behavioral, cognitive, and motivational approaches. - Provides primary care providers and staff regular and concise feedback on the status of referred patients, to include recommendations for team-based care. - Implements psychoeducational classes to target high frequency diagnoses found in the primary care clinic. - Documents information in the electronic health record during patient encounters. PERFORMS OTHER DUTIES AS ASSIGNED.
