This position provides virtual/tele-health consults to adult Neurology patients across the state of Indiana. Collaborate remotely with a team that includes three Community Medicine Neurologists based at IU Health North Hospital in Carmel, Indiana.
Position Details:
- Full-Time, Days (REMOTE)
- Nine 4-hour clinic sessions per week
- 8:00am-5:00PM
- No weekends, holidays, or call
- Provide clinical care and documentation pertaining to adult patients established with the Neurology team.
Qualifications:
- ACNP, AGNP, FNP or PA certification required.
- Clinical Neurology experience required.
Key Responsibilities:
- This position provides virtual consults to adult Neurology patients ranging in ages eighteento geriatric.
- Primary focus will be providing clinical visits for established adult Neurology patients including medication management, specialty-care referrals, and scheduling for laboratory and radiologic testing.
- Advanced Practice Provider will also triage established Neurologypatients that are discharged from the hospital.
- The ideal candidate must feel comfortable working independently and collaborating with an interdisciplinary team of Physicians, nurses, and other care team members.
