Responsibilities:

• Provide primary care to patients while receiving rehabilitative care in our prestigious specialty hospitals
• Collaborate with treatment team members for medication management
• Implement an individualized plan of care for patients' goals
• Complete acute, routine follow up, and discharge visits, and transitional care
• Make recommendations on DME, home health services, or outpatient services post discharge

Qualified applicants should have or be able to obtain:

• Active Virginia medical license and DEA
• Active Medicare and Medicaid number

Competitive compensation, with an estimated salary range of $100,865 to$121,037 annually with the opportunity to earn incentives.
