Job Details

• One day a week:15 -20 patients for the day
• MD or DO
• Compensation is fee for service (FFS); expected compensation is estimated income range of $100,000 to $120,000 annually with no cap on productivity income potential

Responsibilities

• Drive high-quality, safe, and cost-effective patient care in post-acute carecommunities
• Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital
• Medical director leadership
• Collaborate with other clinician interdisciplinary teams and follow CMSguidelines
• Work closely with hospice, palliative care, and home care agencies for safepatient transitions
• Ensure timely documentation and billing through our EMR (Gehrimed) meetingMedicare guidelines

Qualifications

• Board certified in internal medicine (IM), family medicine (FM), or emergency medicine (EM)
• Willing to train experienced candidates as well
• Licensed in State
