Geriatric Nurse Practitioner

Job Description

Park Nicollet Home-Based Senior Care is looking for a full-time (0.8-1.0 FTE) Geriatric Nurse Practitioner to join our team. Being a part of our team means you will have an impact on the care that our patients receive every day.

As a Park Nicollet Home-Based Senior Care Clinician, you will work collaboratively to provide primary health care for patients in their homes; provides care coordination to achieve patient centered, high quality and cost-effective care across the continuum; and provides nursing leadership in defining and achieving program goals in a changing health care environment. This position works as part of an interdisciplinary team including Registered Nurses and Social Workers. The position is M-F 8am-5pm (no weekends).

REQUIRED QUALIFICATIONS:

  1. Master’s degree in nursing or public health nursing or equivalent education and experience.
  2. Certification as Nurse Practitioner (Adult/Geriatric/Family) by a national certifying body (ANCC, AANP, etc.).
  3. Nurse Practitioners beginning practice after July 1, 2014 must practice for at least 2080 hours within the context of a collaborative managed setting where nurse practitioners and physician work together in a hospital or integrated clinical setting.
  4. Current Minnesota RN license.
  5. Current Minnesota APRN license.
  6. Approved or eligible for approval by the MN Board of Nursing for prescription writing privileges.
  7. 3+ years experience working with seniors in a clinical setting.
  8. Must be able to provide own transportation for local travel. May provide service to patients at multiple clinic locations and/or hospital sites.

PREFERRED QUALIFICATIONS:

  1. Knowledge of Medicare Guidelines preferred for those assigned to the transitional care settings.
  2. Minimum of one-year experience working in acute care preferred for those assigned to the transitional care settings.
  3. Home care experience preferred for those assigned to the long-term care/assisted living settings.
