What You'll Do

• Lead Medication Therapy Management (MTM) programs to optimize patient outcomes and enhance medication adherence
• Provide expert clinical consultation to our primary care providers and care teams
• Manage our Anticoagulation Clinic to ensure safe and effective warfarin therapy
• Conduct comprehensive drug utilization reviews to prevent adverse events and drug interactions
• Educate and counsel patients on proper medication use, side effects, and adherence strategies
• Collaborate with interdisciplinary teams to develop personalized care plans
• Support quality improvement initiatives focused on medication safety and cost-effectiveness.

What We're Looking For

• Doctor of Pharmacy (PharmD) from an accredited program
• Active Florida Pharmacy License in good standing
• Minimum 1 year retail pharmacy experience
• Strong knowledge of federal and state insurance programs (Medicare/Medicaid experience preferred)
• Excellent communication and interpersonal skills
• Detail-oriented with strong organizational abilities
• Ability to work independently and as part of a multidisciplinary team

Preferred Qualifications

• Pharmacy residency training
• Managed care experience (HMO, MSO, PBM)
• Immunization certification
• Experience working with geriatric populations
