SUMMARY:
We are seeking a Nurse Practitioner to work at our Family Practice office located in California, Maryland. This role requires strong clinical skills, excellent communication abilities, and a compassionate approach to patient care. We are a growing practice, and we are looking for an FNP who is excellence-minded, service-driven, principled, and forward-thinking. As an FNP, you will provide comprehensive primary care services to patients of all ages, focusing on health promotion, disease prevention, and management of chronic illnesses.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provides primary medical care for a dedicated panel of patients, including diagnosis and treatment of acute and chronic illness according to evidence-based standards.
- Obtains clinical histories, conducts appropriate physical examinations and assessments, orders diagnostic testing and formulates treatment plans.
- Conducts routine health examinations and evaluations and makes recommendations for preventive health maintenance and screening tests as appropriate.
- Documents appropriate clinical information in the electronic health record in a timely manner, completing documentation of clinical encounters and locking progress notes within a specific timeframe.
- Reviews documents including outside records and consultant reports and summarizes information in the medical record.
- Determines need for additional services including but not limited nursing, social services, health education, and referral for specialty care.
- Orders and is responsible for the dispensing and administration of certain medications on-site.
- Refers patients to hospitals for emergency care and admission when needed and communicates with hospital staff as appropriate.
- Authority for coordinating and making decisions regarding patient care.
- Participates in quality improvement/patient safety projects.
- Supports providers in phone follow up, in-basket message management/triage, patient paperwork.
- Coordinates care for a group of primary complex patients to achieve clinical and organizational outcomes.
- Responsible for other clinical duties as assigned.
KNOWLEDGE AND SKILLS:
- Familiar with anatomy, pharmacology, pathophysiology, clinical medicine, and physical diagnosis.
- Knowledge of diagnosing and treating medical problems and developing care plans.
- Requires effective use of resources and cost awareness.
- Ability to incorporate into practice theoretical concepts and advanced clinical skills with patients, families, and interdisciplinary health care team members.
- Analytical ability necessary to diagnose patients and to develop and implement plans of action for long and short-term management of acute and chronic health.
- Ability to maintain personal composure in high stress situations.
- Interdisciplinary health care team to enhance collaboration and problem-solving.
- Ability to communicate effectively, both verbally and in writing, in order to provide information to patients and health care personnel.
- Ability to demonstrate a high level of interpersonal skills required to interact with patients, patients' families/visitors and clinical staff.
- Ability to perform with a high degree of accuracy and with meticulous attention to detail.
- Ability to demonstrate self-direction and initiation.
- Ability to work independently and as a part of a team.
QUALIFICATIONS:
- Graduation from an accredited Nurse Practitioner program with current state licensure.
- 3-5 years of experience in or a related field is highly preferred but not required.
