• Assist the Program Director with efforts to actively promote the Program to prospective participants and health care professionals as needed.
• Provide nutrition education, counseling, and coaching to participants about the implementation of the Program's nutrition guidelines.
• Deliver nutrition lectures/sessions and cooking demos/classes according to Program guidelines.
• Conduct a nutritional assessment for nutritional adequacy on each participant.
• Communicate clinical issues to the nurse case manager and medical director.
• Participate in the development of the interdisciplinary treatment plan to ensure the quality and clinical effectiveness of the program, and the safety of each participant.
• Implement the Nutrition element of the Program intervention. Assist the Program Coordinator with efforts to actively promote the Program to prospective participants and health care professionals as needed.
• Provide nutrition information for the community that may include public speaking, news releases, and other forms of publication.
• Conduct a nutritional assessment for nutritional adequacy on each participant. Collaboratively sets goals with participants in order to achieve adherence to Program guidelines. Document's the participant's response to education, counseling and coaching.
• Develop meal plans, taking both cost and clients' preferences into account.
• Evaluate the effects of meal plans and change the plans as needed.
• Promote better health by speaking to groups about diet, nutrition, and the relationship between good eating habits and preventing or managing specific diseases.
• Keep up with or contribute to the latest food and nutritional science research.
• Write reports to document patients' progress.

Licensure / Certification / Registration:

• Registered Dietitian/Nutritionist credentialed from the Commission on Dietetic Registration obtained
  prior to hire date or job transfer date required.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's
  degree/Technical degree OR 4 years of applicable cumulative job specific experience required
