  • Specialty: Family Practice - Primary Care
  • Schedule: 1.0 FTE, with 36 Patient Contact Hours; 4 hours of admin time per week. Clinic hours are 7:00am to 5:00pm. Providers work +2 hours a week (outside of 8:00am to 5:00pm).
  • Call Schedule: On-Call provider takes nightly/weekend/holiday call, 1:7.
  • Practice Detail: Ascension Medical Group in Chilton, Wisconsin is looking for a Family Practice Physician to join our elite team (2 MDs and 5 Nurse Practitioners-fully staffed). Our clinic is located on the campus of Ascension-Calumet Hospital.

Responsibilities

Ascension-Wisconsin is looking for a Board Certified/Board Eligible Physician for our Family Medicine Team.

This is a great opportunity for the right candidate to partner with Ascension-Wisconsin in Chilton to provide medical services to our patients.

Responsibilities:

  • Evaluates and treats patients with appropriate medical diagnostic and treatment skills.
  • Recommends, participates in, and works to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.
  • Accepts patient referrals from other physicians. Acts as admitting, attending and/or consulting physician for patients who require hospitalization for primary medical diagnoses, depending on needs of patients, medical staff and hospital.
  • Participates in academic programs (e.g. medical education and research) as requested.
  • Provides appropriate documentation that meets insurance company requirements; appeal all denials in a timely manner.
  • Participates in inpatient and outpatient consultation for specialty, which may include on call schedules.

Requirements

Licensure/Certification/Registration:

  • One or more of the following required:
    • Physician MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.Licensure is dependent on state associate works in. Verified by local Credentialing Office.
    • Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.Licensure is dependent on state associate works in. Verified by local Credentialing Office.
  • Advanced Life Support credentialed from the American Heart Association (AHA). Verified by local Credentialing Office preferred.

Education:

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
