Opportunity Overview:- Working in the town of Vancouver, WA
- 15 -20 patients per day
- This is a part-time position working 1 day a week
- Advanced practice clinician (APC)
- Compensation and Benefits: Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated income range of $41,000 to $49,000 annually with no cap on productivity bonus income potential. This part-time role is benefit eligible.
Responsibilities:- Drive high-quality, safe, and cost-effective patient care in post-acute carecommunities
- Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital
- Collaborate with other clinician interdisciplinary teams and follow CMSguidelines
- Work closely with hospice, palliative care, and home care agencies for safepatient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meetingMedicare guidelines
Qualifications:- Board certified FNP, AGACNP, ACNP, AGPCNP, or PA-C
- Willing to train experienced candidates as well
- Licensed in state
