Nurse Practitioner – Outpatient Palliative Care | Asante Southern Oregon

Asante is seeking a compassionate Nurse Practitioner to deliver holistic, patient-centered palliative care in Southern Oregon. This role focuses on supporting individuals with serious or life-limiting illnesses in outpatient and home settings, providing comfort and dignity during critical times.

Responsibilities:

• Conduct assessments, manage symptoms, and create personalized care plans.

• Prescribe medications, order tests, and educate patients and families.
