Family Medicine Physician Job at ICAHN Rural Recruitment in Paris, IL

Horizon Health has an opportunity for a Primary Care Advance Practice Provider to join a growing Family Medicine practice in Tilton, IL.

We are a rural organization that includes a critical access hospital, extensive outpatient services, and rural health clinics, but much larger in scope and size than typical rural organizations. The Advance Practice Provider (APP) will provide primary care for our patients at our new location in Tilton, IL. We are looking for an APP candidate with experience, compassion, and strong clinical skills. This APP will assess patients regarding their conditions, order and interpret diagnostic tests, and work with patients and families to establish care plans.

Family Practice Information:

• Current license as an Advanced Practice Registered Nurse in the State of Illinois.
• Cardio-Pulmonary Resuscitation-BLS.
• Preferred three (3) years of Advance Practice Provider experience in Family Medicine or specialty area.
• Compassionate patient center approach to care
• Excellent time management skills
• Exceptional communication skills
• Ability to work independently.
• The clinic will operate Monday through Friday and will allow for walk-in patients
• Horizon Health is staffed with 20 primary care physicians and providers, with specialty support in behavioral health, urology, urogynecology, general surgery, orthopedics, spine, pain, dermatology, podiatry, wound care, cosmetics, endocrinology, and cardiology.
• Additionally, we have 11 visiting specialists that see patients in the clinic each month. The hospital has a 24/7 hospitalist service and convenient care for patients.
• We have a provider friendly EMR.
• In 2023, we completed expansion projects including adding an outpatient dialysis center, home health services, pre-anesthesia clinic, and innovative radiology updates.
