This is a full-time Nurse Practitioner/Physician Assistant role where you’ll provide direct care to geriatric patients at an average age of 85. Typically, you’ll see 4–6 patients per day for 45 min-1 hr visit. You will be conducting follow-up visits derived from hospital appointments, checking their medications, ensuring the patient is adhering to their treatment plan. Additionally, you will review medical records, conduct bandage changes, address wound care, and may also draw blood for labs or insert IV fluids.
The point of your visit is to provide treatment for whatever the patient needs and fill in the gaps until the member can see their PCP. In total, you’ll have a panel of 250 patients within a particular geography, which will enable you to build a relationship with them. Your patient panel is based on proximity to your home and would be comprised of members who are primarily Medicare Advantage patients.
Although you are providing 1:1 care to patients, you do have an interdisciplinary team to collaborate with that consists of Physicians, Nurses, Social Workers, Pharmacists, Dietitians, Behavioral Health, Administrative Support and other professionals.
Day-to-Day Outline: Each morning you will have a quick huddle to discuss the day (either by zoom, phone or at the local office). Then you’ll go to your patient appointments to provide care. You’ll assess their needs and their medical, social, psychological, and environmental safety. You’ll develop and update their care plans and document that info electronically. You’ll also communicate directly with their PCP to address condition changes you’ve noticed or potential medication changes.
Schedule: Mon-Fri 8am-5pm (local time zone) with a 30-minute lunch. Nurse Practitioner/Physician Assistant: 4 patient-facing days a week and 1 admin day.
Admin is one 1/2 day a week on Thursday morning for IDT from 8:30am-12:30pm. Then they do patient visits in the afternoon. IDT is market training, announcements, etc...
