Job Overview
The Nurse Practitioner for the Center for Older Adult Health at TVR-Tice Valley Rossmoor Clinic provides comprehensive and collaborative care for older adults diagnosed with Alzheimer’s or other dementias. They will undergo training provided by the UCLA Alzheimer’s and Dementia Care (ADC) Program to offer comprehensive, coordinated, person-centered care for patients with dementia. The program’s goals are to maximize patient function, independence, and dignity, minimize caregiver strain and burnout and reduce costs through improved care. The dementia specialist NP will work under the supervision of the clinic’s medical director, provide consultative services and co-manage patients with the patient’s primary care team. They will provide detailed assessments to diagnose and treat unique health issues and complexities associated with dementia. They will create treatment plans and execute strategies to stabilize the patient’s physical and mental health threats and improve overall well-being. They will work closely with an interdisciplinary team to coordinate care and to ensure comprehensive, high-quality care with the goal of maintaining quality of life accordant to the patient’s wishes while optimizing patient outcomes.
Job Responsibilities:
- Complete the required Dementia Specialist training provided by UCLA ADC program.
- Perform structured in-person initial needs assessments of patients and their caregivers
- Create and implement individualized dementia care plans based on needs assessments and monitor and revise care plans, as needed.
- Perform extensive assessments aligned with 4 Ms: Mentation, Mobility, Medications, What Matters
- Conduct in-depth, dementia-centered physical evaluations.
- Evaluate patient mental health, nutrition and external support networks.
- Educate patients and caregivers on coping skills to handle dementia-related health issues.
- Create treatment plans and execute strategies to stabilize patient health, mitigate physical and mental health threats and improve overall well-being.
- Work closely with the medical director, IDT (interdisciplinary team) not exclusive of PCPs, social workers, RNs, care coordinators, pharmacists, behavior health, specialists and community groups to coordinate treatment plans.
- Maintain communication and contact with patient’s primary care team and inpatient team when patient is hospitalized.
- Perform transitional care including post discharge follow ups from hospital and skilled care facilities.
- Follows evidence-based, national and internal practice guidelines and policies.
- Attends required follow-up monthly and quarterly ADC training meetings as well as IDT meetings and any pertinent administrative committee meetings as needed.
Qualifications:
Required:
- Graduate of an NP-focused Master’s or Doctoral nursing program with national NP Board Certification
- Current RN and DEA license or eligibility in California
- At least 3 years of experience in general adult medicine, ideally in geriatric medicine.
- Must provide references focusing on quality, excellent patient centered-care, bedside manner and professional demeanor and collaboration in multidisciplinary team environment
- Must be willing to complete the ADC training program: 12.5-hour, 24-module online curriculum available through the Gerontological Advanced Practice Nurses Association (GAPNA), 6.5-hour virtual clinical skills training, Zoom training with a dementia expert and group telephone training through the implementation phase.
- Must be physically able to perform all required duties in the clinic setting.
- Adhere to evidence-based, national and internal practice guidelines and policies.
- Comfortable taking care of frail older patients with multiple comorbidities and demonstrate empathy, compassion, and respect for the unique needs of elderly patients
- Excellent communication skills with ability to adjust communication styles when working with patients, their families and colleagues.
- Flexibility and willingness to pivot to various functions within the realm of providing geriatric care in the clinic setting.
- Must be compliant with chart and EMR compliance policies.
Desired:
- Highly organized, strong time management skills and flexibility to manage multiple timelines and activities.
- Understands job responsibilities may extend beyond clinic hours for completion of chart notes and EMR in-basket items.
- Provides after hours call coverage when needed.
- Willingness to play a key role in driving clinical initiatives related to the care of the aging dementia population.
- Experience with Epic EMR and performing telehealth video visits.
- Experience or strong desire to work as a member of the care team to meet the challenge of caring for an aging vulnerable patient population.
