PURPOSE OF POSITION
The primary purpose of the Hospice & Palliative Care Physician is to review, coordinate and oversee the management of medical care for all patients in the Hospice and Palliative Care program.
RESPONSIBILITIES & EXPECTATIONS
- Provides guidance to the interdisciplinary team (IDT) and Ethics Committee in the resolution of conflict and ethical issues
- Utilizes appropriate resources to maintain knowledge and skills for hospice and palliative care in all settings
- Participates in Quality Assessment and Performance Improvement (QAPI) program
- Provides physician representation at and participation in interdisciplinary group meetings
- Acts as a resource to hospice and palliative care staff, patients, family members, and physicians regarding pain and symptom management
- Acts as a liaison to physicians in the community, and provides for consultation and education to colleagues related to admissions criteria for hospice and palliative care
- Ensures appropriate educational experiences for visiting students and residents on area professional programs
- Participates in continuing education for all staff providing direct patient care
- Collaborates with the attending physician regarding the palliation and management of the patients with life-threatening or terminal illnesses
- Assists the IDT in developing and implementing a plan of care that is coordinated with the attending physician
- Provides coverage and support to hospice staff for provision of medical services in the absence of the attending physician and after normal business hours
- Provides medical management, in consultation with the attending physicians, regarding pain and symptom management
- Performs home visits and visits for inpatient hospice services related to patient assessment and care as needed
- Performs palliative care consult visits in facilities or patient homes as needed
- Meets requirements to maintain professional licensure
- Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community
- Demonstrates understanding of team approach and communication
- Provides timely and appropriate response to internal and external customer needs
- Complies with agency infection control and safety policies
- Protects patient health information according to HPPA privacy rule and related Aultman policies

- Supports strategic organizational goals
- Abides by the Aultman Code of Conduct at all times

QUALIFICATIONS
- Completion of medical education and relevant residency training required. Fellowship in Palliative Medicine preferred
- Work experience in internal medicine, geriatrics, palliative medicine or other related field of practice required. Work experience in a certified hospice program preferred.
- Physician licensure by the State of Ohio as a doctor of medicine or osteopathy required. Board certification in hospice and palliative medicine preferred.
- Demonstrated knowledge and well-developed skills in hospice philosophy, medical aspects of hospice and palliative care, including recent research, trends in hospice and palliative care, and pain and symptom management
- Valid Oho Driver’s License
- Must have own transportation and provide proof of current liability insurance
- Basic computer skills
WORKING CONDITIONS:
- Hours of operation including occasional overtime, on-call or off-shifts (evenings and/or weekends) scheduled as necessary.
- Subject to frequent interruptions and changes in priority of duties throughout the day.
- May be required to travel in inclement weather
- Subject to emergency and other crisis situations
- Hazardous Exposure Category 1
- Sitting/standing/moving about during working hours (see attached Physical Requirements Addendum for details)
