Presbyterian Healthcare Services, a large and thriving healthcare system in New Mexico, is seeking an experienced Geropsychologist / Psychologist Geriatric Medicine to join our Medicare Advantage Primary Care Clinic team in Albuquerque. Our Medicare Advantage Primary Care Clinics are designed to provide specialized care to aging adults by integrating behavioral health services with primary care. These clinics offer a comprehensive, team-based approach that includes coordination of medical, behavioral, and social services. Our goal is to improve the health outcomes of our Medicare Advantage members through focused, accessible, and integrated care.
As a member of our team, you’ll play an important role in ensuring our aging adult patients receive the support they need for both mental and physical well-being. The PCBH model allows for quick, effective interventions in a primary care setting, where you’ll collaborate closely with other providers to offer well-rounded care to our patients.
