Responsibilities
- Provide comprehensive outpatient primary care to a diverse patient population in a primary care setting.
- Conduct thorough health assessments, including physical exams, medical histories, and diagnostic tests.
- Diagnose and manage acute and chronic conditions, prescribing medications and therapies as appropriate.
- Develop and implement individualized care plans in collaboration with patients and their families.
- Collaborate with a multidisciplinary team, including physicians, nurses, and specialists, to ensure high-quality care delivery.
- Provide preventive health services, including immunizations, screenings, and counseling on lifestyle modifications.
- Utilize EPIC for accurate and timely documentation of patient assessments, diagnoses, care plans, and treatments.
- Educate patients and their families about managing chronic conditions, medications, and maintaining overall health.
- Support the implementation of evidence-based practices to improve patient outcomes.
- Participate in quality improvement initiatives to enhance patient care and satisfaction.
Minimum Requirements
- Master's degree in nursing from an accredited program
- Valid and active Nurse Practitioner license in the state of Virginia
- Certification as a Nurse Practitioner (NP)
- Proficiency in using electronic health records (EHR), particularly EPIC
