The Community Health System (Community Medical Centers "CMC") is a locally owned, not-for-profit, public-benefit organization based in Fresno, California. Community is the region's largest healthcare provider and private employer. We operate a physician residency program with one of the nation's top medical schools – the University of California, San Francisco. Community is home to the only Level 1 Trauma Center and comprehensive burn center between Los Angeles and Sacramento. We also serve as the area's "safety net provider." Clinical or translational research interests are encouraged and supported.
Accountabilities
- Enter, adjust, and/or modify orders and dosages based on guidelines on provider’s behalf
- Troubleshoot treatment and therapy plan issues received from Oncology physician team
- Collaborate with providers during drug shortages and make proper modifications to treatments
- Review, implement, and assist in the creation of clinical trial protocols
- Coordinate with in-patient team on proper transitions of care
- Create treatment plan protocols based on provider’s request
- Consult patients on medications (chemotherapy, oral medications, injections, etc.)
- Answer clinical questions for physicians, APPs, medical assistants, and staff
- Review all treatment plans and therapy plans for accuracy and completion
- Utilize reports to review, renew, and discontinue treatment plans on a weekly basis
- Optimize Electronic Medical Record System use and improve workflow for all providers
- Participate in the Oncology Pharmacy and Therapeutics team as well as the Beacon Workgroup team in protocol review and implementation
- Precept pharmacy students
- Resolve billing issues on provider’s behalf
Qualifications:
Licenses and Certifications
- A current license to practice as a Registered Pharmacist issued by the California State Board of Pharmacy (Required)
- A current license to practice as an Advanced Practice Pharmacist issued by the California State Board of Pharmacy (Preferred)
- At least two years’ experience in oncology (Preferred)
