Presbyterian Medical Group seeks a BE/BC Primary Care, Family Medicine or Internal Medicine or Geriatric Medicine trained physician to join our established practice in Albuquerque, New Mexico. This is a full-time position with our employed provider group, Presbyterian Medical Group. The Complete Care Home Program Team provides primary and urgent medical care for patients in the home environment including comprehensive medical care to patients with acute and chronic illnesses and serves as admitting Provider for the Hospital at Home program. The Medical Provider leads and works collaboratively with an interdisciplinary team including nurses, social workers, case managers, skilled therapists, aides, and clerical staff to provide continuous care to patients.

Practice Highlights:
Provide care in a Home-Based setting or Skilled Nursing Facility or Outpatient Clinic
24-hour Triage RN support for patients
Average volume varies based on daily care provided.

Must Be BE/BC in Family Medicine, Internal Medicine or Geriatric Medicine (Skilled Nursing Facility Preferred)
Patient-focused and willing to collaborate and work in a team environment
Ability to obtain a medical license to practice in the state of New Mexico
