Reports to: VP of Provider Services

Summary: At American Family Care, it is our mission to provide the best healthcare possible in a kind and caring environment while respecting the rights of all patients, in an economical manner, at times and locations convenient to the patient.
Our Regional APP Directors are essential in delivering upon our mission through the delivery of outstanding clinical care for 70-80% of their time with 20-30% of this time dedicated to administrative duties.
Delivering the Best Healthcare Possible
· APP Director Duties
o Utilizes site-specific and individual-specific data to help drive change
o Monitors lab and x-ray review process and ensures they are completed
o Works with leadership to develop clinical pathways to reduce unwarranted practice variation and measure for success
o Completes quality of care incident reports as assigned and utilizes findings to help individuals, sites, and the organization improve
o Develops a project per year to improve quality, safety, service and value
o Mentors newly hired APPs
o Supports the peer review process
· Clinical Duties
o Assesses patient needs within an urgent care and primary care setting
o Appropriately triages patients to differentiate the patient needs
o Evaluates patients in a timely manner, putting patient care above clerical duties
o Performs a thorough history and physical exam on each patient
o Creates an individualized, evidence-based plan of care
o Appropriately consults the supervising or back-up physician
o Effectively manages private, occupational medicine, and workers’ compensation patients
o Follows evidence-based standards for ordering laboratory, radiology and other diagnostic studies
o Uses evidence-based resources (i.e. Up to Date) for treatment options
o Orders and prescribes appropriate medications based upon best practice standards
o Appropriately consults and refers to specialist
o Performs procedures including, but not limited to, local anesthesia, digital blocks, wound management and closure, foreign body removal, incision and drainage, manages orthopedic appliances, & fracture management.
o Complies with current rules and regulations by the governing bodies of state for
o Educates patients regarding the disease process and discharge plan
o Approaches the vocation of health care as an opportunity for lifelong learning
o Completes training requirements, including CME requirements for specialty
o Serves as a resource in educating other staff members
Respecting the Rights of All of Our Patients
· APP Director Duties
o Develops department budgets and capital budgets with operational partners
o Helps to ensure appropriate volumes of patients being seen at the clinics
o Ensures appropriate documentation is completed for visits by working with RCM
o Supports providers in completing coder feedback
o Works across providers to ensure “visits on hold” are complete
· Clinical Duties
o Treats every patient professionally and compassionately
o Utilizes the platinum rule by treating others the way that they want to be treated
o Communicates with clinical staff regarding patient condition and plan of care
o Does not discuss details of business with patients
o Does not label patients (i.e. drug seeker)
o Maintains patient confidentiality
Creating a Kind and Caring Environment
· APP Director Duties
o Mentors, coaches and develops members of the team utilizing qualitative and quantitative data
o Completes performance reviews
o Partners with recruiting to find talent and build teams
o Leads meetings and staff meetings as assigned
o Participates and leads in committee and work group meetings
o Develops educational programming and support as assigned
· Clinical Duties
o Arrives to work on-time
o Adheres to the dress code HR- 500 policy
o Professional and respectful of staff
Caring for Patients at Times and Locations Convenient to the Patient
· APP Director Duties
o Ensures DOT training and testing is completed on new hires within 90 days
o Provides afterhours call as needed for clinics
· Clinical Duties
o Continues to see new patients that may arrive until the time the office closes
o Efficiently evaluates and treats patients in a timely manner while still maintaining high quality and safety as well as an outstanding experience.
Delivering Care in an Economical Manner
· APP Director Duties
o Develops department budgets and capital budgets with operational partners
o Helps to ensure appropriate volumes of patients being seen at the clinics
o Ensures appropriate documentation is completed for visits by working with RCM
o Supports providers in completing coder feedback
o Works across providers to ensure “visits on hold” are complete
o Helps to ensure appropriate volumes of patients being seen at the clinics
· Clinical Duties
o Accurately documents the patient visit and follow up care to communicate, mitigate risk, and ensure appropriate billing and coding
o Completes charts within 7 days of shift
Basic Qualifications
· APP Director Qualifications
o 5+ Years of Clinical Experience in Primary Care, Urgent Care, or Emergency Medicine
o 2+ Years of Experience in Previous Medical Leadership Role
o Full-time employee
o Ability to Travel to Sites Across a Region as Needed
· Clinical Qualifications
o Actively Board Certified and able to see all patients across the lifespan
o Licensed provider in the state of practice
o Does not have an encumbered license
o Ability to work well independently evaluating and managing patients
o Ability to work well in a fast-paced environment
o Friendly and customer-service oriented
o Able to work well in team environment
o DOT certification or the ability to be certified within 90 days of employment
Preferred Qualifications
o CPR training
o Performance Improvement Training and Experience
o MBA, MHA or Other Advanced Degree
