Join Elica Health Centers as a Licensed Clinical Social Worker (LCSW) Today!

You will be responsible for helping to deliver mental and behavioral health services to patients under an Integrated Behavioral Health program.  Your duties will include mental health & substance abuse assessment, integrated health care screenings, brief solution-focused therapy, referral, and support services.  Services will be provided to clients, families, and others from diverse backgrounds.

Your Direct Patient Care Activities:

  • Assist patients in addressing acute mental health needs, behavioral impacts on physical health, management of chronic conditions and promotion of well-being
  • Collaborate and coordinate with other members of EHC Care Team, particularly the client s PCP, to provide holistic, integrated health coverage in order to improve the outcome and well-being of patients
  • Provide short-term, structured, solution-focused, evidence-based therapy as part of an appropriate treatment plan, which may include individual and/or group therapy, and possible coordination of treatment for participants with co-occurring disorders
  • Provide and interpret mental health screenings and preliminary assessments for clients who have been referred by primary care providers (PCPs)
  • Provide brief behavioral health screenings, preventive behavioral health care, and proactive identification of possible behavioral health needs within the patients of the medical and dental clinics
  • Recommend and/or facilitate referrals to other services, such as psychiatry, substance-abuse treatment, long-term therapy, urgent/crisis care, specialty care, and community resources, as needed
  • Participate in Warm Hand-Offs as requested by medical providers and facilitate Warm Hand-Offs to medical providers as needed
  • Provide crisis intervention as needed
  • Maintain accurate and timely records of activities, encounter notes, and services provided to each client
  • Advocate for patients and work to resolve individual patient barriers in order to facilitate and encourage follow-up and collaborative, interdisciplinary care
  • Participate in and/or facilitate EHC Care Team meetings
  • Other duties as assigned

Your Program Oriented Activities: 

  • Attend monthly staff meetings
  • Participate and contribute to monthly in-service meetings
  • Abide by and promote agency administrative policies
  • Maintain continuing knowledge and practice of competence by participating in continuing education opportunities facilitated by professional organizations
  • Participate in performance evaluation at scheduled intervals throughout employment

Your Education and Experience to include:

  • Master s degree in Social Work from a university accredited by the Council on Social Work Education, and possession of a current Clinical Social Worker license issued by the California Board of Behavioral Sciences; OR,
  • Doctorate degree in Psychology and possession of a current Psychologist license issued by the California Board of Psychology
  • Current CPR (BLS) certification
  • Experience in short-term, evidence-based, solution-focused counseling; creation of treatment plans; psychosocial assessment; individual therapy; group therapy; cognitive-behavioral therapy; motivational interviewing; trauma-informed care; and crisis intervention
  • Knowledge of psychiatric, psychosocial, ethical, legal and privacy issues; substance misuse/abuse and chemical dependency assessment and treatment; understanding of current insurance benefits and restrictions in the area of mental health; knowledge of DSM-V; and treatment issues concerning adolescents and minors
  • Knowledge and understanding of human behavior and communication, psychotherapy, case management, and the influence of cultural and spiritual values in counseling practice.
  • Knowledge of modern office equipment
  • Familiarity with federal, state, and county regulations for FQHCs, as well as knowledge of upcoming changes to Medi-Cal coverage and requirements are preferred.
  • Familiarity with integrated behavioral health models within a community healthcare system; experience practicing in such a setting preferred

  • Ability to maintain confidentiality and HIPAA requirements
  • Ability to maintain accurate and current records and files
