Job Description

Position Summary

Swope Health is seeking a compassionate and knowledgeable Qualified Substance Use Disorder Counselor for Swope Health Imani House.  The Qualified SUD Counselor is responsible for providing all treatment services for Imani Clients.

PRIMARY RESPONSIBILITIES:

  1. Lead groups of a therapeutic or psycho-educational nature as part of treatment services.
  2. Provide ongoing treatment services as described in the program’s procedure manual to target populations.
  3. Develop individualized treatment plan based on all evaluation and assessment procedures as outlined in program procedure manual.
  4. Provide individual therapy for clients.
  5. Complete required paperwork according to organizational timeliness.
  6. Complete required billing functions accurately and consistent with internal and funder guidelines to support reimbursement for services.
  7. Connect clients with referrals as needed.
  8. Must complete all annual trainings.
  9. Meet productivity requirements.
  10. Participate in Multidisciplinary treatment planning meetings.
  11. Collaborate with other providers and/or programs to best meet client needs.
  12. Provide family counseling and consultation as clinically indicated
  13. Attend ADA department and Swope Health Services required meetings.
  14. Maintain current credentials while employed.
  15. Other duties assigned by supervisor.

Education/ Qualification Requirements 

  1. A graduate of an accredited college with a Master’s Degree in Social Work, Counseling, Psychology or other closely related field with at least two (2) years of experience in the treatment of rehabilitation of substance abuse. Preferred
  2. A graduate of an accredited college or university with a Bachelors Degree in Social Work, Counseling, Psychology or       other closely related field with three (3) years of experience in the treatment of SUD. Required
