About Signify Health
At Signify Health, part of CVS Health®, our mission is to build trusted relationships to make people healthier. Our clinicians identify chronic conditions, close gaps in care, and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs. Overall, our clinicians help Medicare Advantage, Medicaid, and certain commercial plan members gain a more comprehensive view of their health – all in the comfort of the member’s home.
About our model
Our network of NPs, PAs, and physicians conduct In-home Health Evaluations (IHEs). These visits complement an annual wellness visit with a primary care provider and focus on overall health, well-being, and potential barriers to care — typically enabling more one-on-one time with a healthcare provider versus a standard office visit. Following a clinician visit, a report is shared with the member’s primary care provider and health plan.
