• Avg LOS: 4.49 days
• Average Encounters per year: 4,000 billed visits annually.
• Rounding: Starts at 8 am with the team which includes social worker/case manager, pharmacist and nursing.

Acuity: CHF, Cellulitis, COPD, Gastro
Subspecialty Support: Top DRGs (excluding behavioral health): 871 & 872 – Septicemia or Severe Sepsis, 291 – Heart Failure and Shock, 189 – Pulmonary Edema and respiratory, 190 & 191 – COPD, 177 – Respiratory Infections, 193 & 194 – Simple Pneumonia
EMR: Altera Paragon (we expect to move to Epic by 1/1/2027)
Ancillary Services: Lab, Imaging, wound care, neurology, cardiology, infectious disease, pt/ot/sp
Experience + Qualifications
