Conviva Care Centers and Elite Health is a subsidiary of Humana, Inc. We are the fastest growing value-based, senior-focused primary care platform in the country. We are currently searching for a Primary Care Physician’s to join our team in Miami-Dade County, Florida.   

Patient-focused care is our ultimate goal:

• You will see typically 12-15 patients per day once your have a full patient panel. 
• We are a Value-Based Care Model not a Fee for Service model or RVU driven.
• You will be a part of an experienced medical team to offer a holistic method of care for seniors. You will be partnered with an Advanced Practitioner and a Care Team consisting of Pharmacist, Behavioral Specialist, Community Outreach, Social Worker, Coach Care Nurse and more onsite.
• 100% Medicare Advantage patients- treating all Medicare Advantage not only Humana patients
